Lafourche Parish Clerk of Court Death Records

Annette M. Fontana serves as the Lafourche Parish Clerk of Court. Her office issues certified death certificates and keeps vital records for the 17th Judicial District. The clerk's office provides records from July 2012 forward. For deaths before that date, you must contact LDH Vital Records at (504) 593-5100.

The clerk's office explains its scope clearly: "The Clerk of Court's Office provides certified copies of birth and death certificates for Lafourche Parish to qualified individuals. The office keeps records of births dating back a century and deaths beginning in July 2012." This is important. If you need a record from before July 2012, the clerk cannot help and will direct you to the state office.

The main office is temporarily located at 200 W. 2nd St., 3rd Floor, Thibodaux, LA 70301. Hours there are Monday through Friday, 8:30 AM to 4:30 PM. Birth and death certificate requests are accepted only from 8:30 AM to 3:30 PM each day. Plan your visit accordingly and arrive before 3:30 PM to make sure your request can be processed that day.

The parish also has two branch offices. The Lockport branch is at the Lockport Civic Complex, 710 Church Street, Lockport, LA, open Tuesdays and Thursdays from 8:30 AM to 4:30 PM. You can reach Lockport at (985) 532-7361. The Cut Off branch is at 131 East 91st St., Cut Off, LA 70345, open Mondays, Wednesdays, and Fridays from 8:30 AM to 4:30 PM. That office number is (985) 632-8111. Branch offices may or may not process vital records requests; call ahead before making the trip.

Under Louisiana Revised Statute 40:41, death records are confidential for 50 years after the date of death. After that window, the records are public. The clerk requires requesters to show proper photo ID and document their qualifying relationship to the deceased before issuing a certified copy of any restricted record.

This 90-day window means very recent deaths are available at the clerk's office quickly after registration.

For deaths registered before July 2012 in Lafourche Parish, the Louisiana Department of Health is your source. LDH Vital Records is at 1450 Poydras St Suite 400, New Orleans, and is open Monday through Friday, 8:00 AM to 3:30 PM. Call (504) 593-5100 for information. The Louisiana State Archives at sos.la.gov provides a free online public vital records index. This tool covers historical death records and lets you search by name, year, and parish.
